Sun River Health never turns anyone away, regardless of citizenship status, health insurance status, or ability to pay.
If you don’t have health insurance, we have a sliding fee scale based on family size and income to make sure you get care at a price you can afford.

If Sun River Health has not already assigned you to a Payment Group, or if your household income has recently changed, you must bring at least one of the following documents with you to your Health Center visit:
- Most recently filed income tax return
- Paychecks stubs from current or most recent job
- Most recent W-2
If you are unable to bring one of these documents, we will help you fill out a one year self-attestation form and see which payment group you qualify for. If you forget to bring one of these documents with you to more than three appointments, you will be charged the full price.
